File a Credentialing Request

The Fund is able to provide Credentialing Requests upon receipt of the following documents:

• PROFESSIONAL LIABILITY CREDENTIALING REQUEST FORM
• AUTHORIZATION FOR TCRMF TO RELEASE INFORMATION
